We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

HOW TO ENROLL
Action Required: All elections must be submitted by 03/29/2023
ENROLLMENT INSTRUCTIONS:
- Log in to askar.ease.com per the instructions you have received from HR
- Log in to Ease per the instructions you have received from your HR administrator
- Follow instructions and enroll in your benefits
- Make sure to save your elections
Remember, this is the only time during the year that you can enroll in coverage or change your benefit elections, unless you have a qualifying event during the year. You have 30 days from the qualifying event date to notify your Benefits Administrator to make the changes. Common life events are marriage, divorce, birth, adoption, and loss in coverage.

You have three options to choose from:
- BCBSM PPO 1500
- In and Out of Network Coverage available.
- Members are not required to have a designated PCP to receive in-network benefits.
- No referral is required to see a specialist.
- BCN POS 1000
- In and Out of Network Coverage available.
- Michigan residents are required to have a designated PCP to receive in-network benefits. In network providers include BCN HMO providers.
- Non-Michigan residents do not need to select a PCP. In network providers include nationwide Blue Card Traditional providers.
- No referral is required to see a specialist.
- BCN POS 3000
- In and Out of Network Coverage available
- Michigan residents are required to have a designated PCP to receive in-network benefits. In network providers include BCN HMO providers

Askar Brands provides those employees who are covered by one of the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provides members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

- Cold/Flu Symptoms
- Sore Throat
- Pink Eye Rashes
- Sinus Infections
- Ear Infections
- Stomach Issues
How Much Does it Cost?
- PPO 1500: Covered 100%
- POS 1000: Covered 100%
- POS 3000: Covered 100%
- POS HSA 6900: Coverd 100% After Deductible (Prior to deductible, member will be responsible for a $59 charge that will apply towards deductible)
- Prescriptions: If you are prescribed a medication during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in and whether your deductible has been met.

Askar Brands dental plan provides comprehensive coverage to help you and your family maintain good dental health. Be aware if you go out-of-network, your costs will be higher.
VISION

Askar Brands offers Vision coverage through Unum to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

Askar Brands provides you with financial protection benefits with Basic Life insurance and Short-Term Disability benefits with Unum. You also have the opportunity to purchase Supplemental Life insurance.
Basic Life/AD&D- Employer Paid (Provided to Managers and above)
You are automatically enrolled in Basic Life and AD&D insurance benefits if you are a manager or above. Your Basic Life insurance benefit is $20,000 (reduced if you are age 65 or older), and the amount of your AD&D benefit is $20,000.
Supplemental Life- Employee Paid
You have the opportunity to enroll in Supplemental Life insurance for yourself, your spouse or your dependent children, which will supplement lost income in the event of death. You must elect Supplemental Life insurance coverage in order to elect coverage for your spouse and/or children.
- Employee: Choose in $10,000 increments up to the lesser of 5x your annual salary or $500,000. Guarantee Issue: $100,000
- Spouse: Choose in $5,000 increments up to 100% of the amount you elect for yourself or $500,000 Guarantee Issue: $25,000
- Child: Choose in $2,000 increments up to $10,000 *Live birth to 6 months, maximum benefit is $1,000. Guarantee Issue: $10,000
Evidence of Insurability (EOI): When you are first eligible (at hire) for Voluntary Life, you may purchase up to the Guaranteed Issue without Evidence of Insurability (EOI). Any amount elected over the GI will require EOI.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short Term Disability- Employer Paid (Provided to Managers and above)
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You are automatically enrolled in Short Term Disability if you are a manager or above. This benefit will provide you 60% of your weekly income (up to $1,000 per week) for up to 26 weeks.
Typical claims would include: pregnancy, injuries, joint, back and digestive disorders.
